REGISTRATION1. Register online with FamilyID.com for EVERY SPORT YOU PLAY and if possible make a donation to the program. Each program is funded from the generous donations of families like you!
2. Turn in a completed Physicians Clearance Form to the Athletic Office. ONCE PER SCHOOL YEAR 3. Take the ImPACT(Baseline Cognitive Test). ONCE PER SCHOOL YEAR - Please see Mr. Davis to set up a time for your ImPACT test. |
